All do all three and love it whenever I am doing whichever one.
I pull off (at that I'm gonna moment), spit, and swallow.
It really depends on what mood I am in. And trust me he doesn't care because he never really knows what one I will do.
My favorite time is when I told him I was going to pull off and he said I am gonna ... and I grabbed his ass and sucked him in deeper. He literally about passed out. hehe
