I'm not sure that 'grab' is the word I'd use exactly but basically the penis moves inside the vagina and the foreskin kinda lags behind a little so that on the inward thrust, the head of the penis is uncovered more and on the outward thrust, the head disappears back underneath the foreskin.

I'd say on both occasions you could have described the sex as 'abnormally vigorous' and in both cases it was I who was in control of the movement which caused the problem. In your standard missionary position where you will be in charge of how deep the thrusts are and of how they are timed, I really don't think you're going to encounter the same problem. Afterall, if you feel any discomfort you'd stop, wouldn't you?
